Dalena Dang, Nurse Practitioner: Dalena Dang joined Parkdale Queen West CHC as a Nurse Practitioner after completing her Master of Science in Nursing at York University.  Prior to her role with Safer Opioid Supply, she worked in emergency medicine and preventive care.  Dalena aims to provide client centred care informed by harm reduction values and principles.

Praneetha Mahindan, Nurse Practitioner: Praneetha Mahindan graduated from U of T with her MN-NP in 2019.  She started her Nurse Practitioner career in acute care and then transitioned to community healthcare in early 2021 during the height of the pandemic to provide healthcare to unhoused folks in the shelter hotel programs.  She now continues to provides primary care and Safer Opioid Supply programs in various shelters through a mobile model of care.  Praneetha is a strong proponent of harm reduction.

Hannah Levitt, Case Manager: Hannah Levitt started at Parkdale Queen West CHC in May 2022 as the Case Manager on the Mobile Safer Opioid Supply program. Hannah graduated with a Bachelor of Social Work from Toronto Metropolitan with an emphasis on community work and harm reduction. Prior to her role with the Mobile SOS program, Hannah worked as a community Case Manager and as a frontline worker in both shelters and supportive housing. Hannah strives to help increase the sense of community and empower clients from a strengths-based, client-centered approach.

Madison Joyce, Registered Nurse: Madison Joyce completed her BScN from Toronto Metropolitan University in 2019 and began working as a Registered Nurse in harm reduction-based primary care & community health roles. Madison is passionate about dismantling barriers to accessing health care, particularly for folks who have historically been stigmatized, marginalized, and traumatized in healthcare contexts.

Chris Sciascetti, Manager of Addiction Medicine & Withdrawal at Stonehenge Therapeutic Community. I have worked at Stonehenge for 16 years in various roles but new to the Safer Supply Program. I am active in the Guelph Community, I am a mom to two active boys and consider myself a lifelong learner.
